Community Care Team

A team of health and social care professionals who work alongside your GP to support you at home. The team includes nurses, therapists, well -being coordinator and social worker, who also have strong links with many health, local authority and voluntary agency teams.

A member of the team will be able to assess your health and social care needs, aiming to help you maintain or regain as much independence as possible, either following a recent episode of illness; discharge from hospital or as a result of an ongoing health condition.  The team will ensure their input is coordinated and regularly discusses with yourself, your doctor and other professional involved in your care.
